Information on the new Patient Navigator Outreach and Chronic Disease Prevention Demonstration.

Communities with significant health disparities and barriers to care will use HRSA funds to evaluate the impact of using non-medical workers known as patient navigators. The navigators will help patients learn more about their chronic diseases, including cancer, and steer them into screening and treatment as needed.
